Field Name	Meaning
Magazine	Please populate this field with the name of the magazine.
Ad or Editorial	Please select whether the file is meant for "Ad" or "Editorial".
Advertiser Name	Please populate this field with the name of the advertiser.
Email	Please populate this field with a valid email address.
Phone	Please populate this field with the phone number.
File Title	Please populate this field with the initials of the magazine for which you are submitting the item, followed by issue month and year, followed by the company name. Example for Chemical Processing, July 2004 issue, ad for ABC Company = CP_0407_abc
Description of file	Please provide any additional instructions here.
File Upload	You are only able to upload one file per entry (pdf, lpeg, gif, doc, xls, zip, sit...). Note: The file name cannot have any spaces or characters in the name. For example, the file name abc_04/04.zip will return an error due to the /. Please remove all spaces or replace them with underscores. You will not see any reference to the uploaded file at this time.
Submit	Once you have completed the above fields, please click on Submit.
